Digital paths to an increased sense of security and a higher quality healthcare.

Challenge

When a citizen needs health and social care at home, there are two different actors responsible for this – the County Council and the municipality. There is a need to create more efficient processes for collaboration and communication between the health and social care providers.

Identify which work processes and information flows around the health and social care recipient. What follows is to identify what type of digital support is needed to strengthen the care and create a digital infrastructure.

Project goal 1: Stockholms läns landsting, Stockholm City and KTH.

Project goal 2: Stockholms County Council, RISE, Ericsson, Stokab, Telia, and KTH.

Pre-study means (500tSEK, support level 50%) granted by Vinnova.

There are many parties that collaborate on efforts to meet the patients’ need for care, including Stockholm County Council, a number of municipalities and private stakeholders. This sets a high requirement for good interaction and conditions for communication across organisational boundaries, while taking into consideration patient and data security. There is also a need to develop existing and new working methods and organisational solutions to facilitate the synchronisation of care services based on the patients’ needs. 

The project focuses on frail patients/users who are suffering from more than one disease (multimorbid). The overall objective is to identify and develop existing and new working methods, as well as technical and organisational solutions that create quality and added value for the patients and help to improve the work environment and cost effectiveness of the welfare services offered.
